My HR50+ has been intermittently sounding good and other times very grainy and thin.

I've swapped tubes, checked bias, tensioned tube sockets, tried different speaker cables into the cabinet.

I believe the amp to be from early 90's. Is there a likelihood it's an output transformer issue?

There always is that possibility of course but it's unlikely.

Before going in too deep, try the following: different cab, run Soldano preamp through external poweramp, run external preamp through Soldano poweramp, run a patch cord directly from fx-send to fx-return.

Good luck !

Is it likely that playing an amp through a blown cab would have created enough stress on the OT to damage it as described? Would the effect been essentially the same as turning the amp on while not plugged into a cab? It is the only thing I can think of as my cabinet had recently went for a shit.

That wasn't in the OP undecided. But yeah, playing out of a defective cab can certainly kill your OT.
